The new application for the Class of 2015 will be available in May, 2012.

You may start your application file now by sending the following to the Admissions Office:

5 Reference forms. Please print 5 copies and have the following complete these forms:

1. Current supervisor
2. Current RN coworker
3. Current RN coworker
4. Current or previous RN coworker
5. Current or previous medical professional coworker (i.e., RN, MD, CRNA, NP)

• Original transcript from each school attended after high school
   
• GRE score 
• Copy of your current Nursing License
   
• Copy of your current ACLS certification
   
• Copy of any other certification (such as CCRN) 

You are not required to send these items in one packet. Paperwork will be kept in an alphabetical file until an application is received. Please make sure your name is on every item sent to the Admissions Office.
